I guess from the Japanese Fukushima nuclear reactor disaster that one of the main lessons that has become obvious was that US designed GE and Westinghouse Boiling water reactor designs (along with the old Soviet RBMK reactors) are not safe and can fail catastrophically. This has been secretly known since they were built but the American public was essentially misinformed about the safety of these designs. i.e. it was known that even an inappropriate use of a candle could potentially lead to a catastrophic nuclear power plant emergency.
