at the moment on my key chain all I carry is some keys, my surf board fins allen key, a princeton tech LED light. I need more! I have looked around for pill holders and small flint and strikers, but have had no success. I am thinking of getting rid of the bulky end of my firesteel, and drilling a lanyard hole.
has anyone got any ideas for other items?

How about a blade of some kind. I know you can get lock knives that come with lanyard holes.

if you want a bulkyer key chain, you could add some tools on it, like the leatherman micra, swisstech ( micro-tech, micro-plus,utili-key,etc. ), sebertech tools ( m2,m3, m4)and those small knifes some knife manufatures make, etc.
And if you really want a flint on your key chain you could get BCB's ranger flint and take the flint part from the rest and attach that to your key chain and use your keys/tools or whatever as your striker...

and you could add a p-38 ( can opener ) on it , a knife sharper and a carabinder to attach your key chain to your trouwser or some other thing...

When you drill that hole in the ferrocium rod, keep a spray bottle of water handy and use it often to cool the rod and drill bit tip. Otherwise there will be a fireworks display of sparks. The water seems to speed up the drilling as well.

I'm not a big fan of bulky key-chains and the stuff I've got on mine right now is probably more than I really want, but so far it's not too uncomfortable.

In addition to my keys, I have:

International Orange Fox 40 whistle
SAK Classic (won it as a door prize at a company golf tournament 5 years ago)
Photon Microlight (red LED)
Key blank with 4 lighter fllints crazy-glued to it
The breakaway lanyard that came with the Fox 40, chainstitched so it's about 4 inches long

I also carry a credit-card sized Fresnel lens and a PSK-type can opener in my wallet.

What about a small compass and matches. You can buy a little key chain container that you can put matches in.

I added my new Squirt P4 and a "key chain" sized first aid kit put out by Adventure Medical Kits (available at REI among other places). This first aid kit comes with some items I like to have along on short hikes (bandaids, moleskin, asprin & ibuprofen, antiseptic wipes, insect bite relief wipes and antihistimine tablets - good in case one has an unexpected reaction to a bee sting, and tweezers).

It really isn't a key chain anymore so I put it all on a mini carabiner and carry it in a coat pocket. Now if I could just find where I put my keys....
