The simplest way to make sure the EMT's find your info card is just attach it to your drivers' license, e.g. with scotch tape. Your DL is the first thing they will look for. I wouldn't bother with fluorescent paper, glow in the dark stuff, or anything like that.

If my memory serves me correctly, we have touch on this idea before, so you may want to run a search for previous threads that had many excellent ideas.
I will however, state a caution that the information sheet not be developed for use on the scene of a trauma or medical emergency to provide information to EMS providers. This forum has a number of EMS providers, so their experiences may differ from mine and their input should also be considered.
By training and protocol (at least in the US), all EMS providers will look for MEDICAL ALERT bracelets/jewelry to learn of medical conditions that may alert us of life-threatening conditions and potentially influence our treatment of the patient. However, (again my experiences) wallets and purses are generally taken by law enforcement agents, they may or may not have the time to let us know of the presence of medical information located within your wallet/purse, before we leave the scene. In a trauma situation, my/our first priorities are always the ABCs, many times I/we do not have time to search through your personal belongings, which can in and of itself bring us legal grief. At the hospital, it is a whole different situation and information cards can provide much needed information for medical treatment and contact next-of-kin.
If you have a medical condition we need to know about, obtain and wear a MEDICAL ALERT device. Pete

I would agree with Pete. As a volunteer EMT-I, I know we are trained to look for the Medical Alert tags, but in trauma cases this just isn't always the case. Most situations involving trauma, an unidentified patient is always admitted either John or Jane Doe until a proper ID is made. Checking a wallet card is done after life-threatening injuries are cared for. Most of the time, as stated, Law Enforcement is the ones making an identification match from info in the wallet. Having been a police officer—I know making ID is sometimes difficult. Using a fluorescent colored card with big letters saying “MEDICAL CARD” would definitely be helpful as opposed to having to “sift” through all other papers to find any info. Normally a driver’s license is used to make ID. You can also check with your local DMV to see if they offer a place on your drivers license for medical personnel to check a medical card in the wallet. CAUTION: Attaching any information yourself to the back of a driver’s license is “altering” the license itself and could create a different problem. Medical Alert tags make a bracelet or necklace, which clearly states "check wallet card." I think they either will make you a card or you can use your own format like you've described. I created my own emergency card in MS Word. It is a bi-fold card the size of a credit card. It has more than just vitals, as I wanted it to be more help than just medical emergencies. I include several phone numbers: kids school, neighbors, friends, out of area family, emergency contacts, local hospitals, trauma center, my HMO, taxi, tow company, bank, travel agent, credit card phone. I also include procedures for emergency contact: call 1)work/home, 2)cellphone 3)in state family, 4)out of state, etc. I list the procedures for using our answering machine and my work VoiceMail to leave/retrieve messages. I list our primary and secondary meeting places. Fire, EQ, CPR notes (these are for review purposes). I also have the STOP acronym and DRABC acronym. Finally, the comfort of a family picture and Isaiah 41:10. All of this is on the front and back of a Bi-fold paper that is laminated with packaging tape. The kids each have one in their school backback, and mom and dad carry one. I do not include PINs or CreditCard numbers. Thanks for the Emergency Card. Easy setup using Microsoft Word. I was able to add some extra information because of the extra space. Just what I was looking for someone to "invent" and share, and you can't beat Free! Thanks again!
