Pen and pencil (and stick eraser and marker). I also geocache so to be without a writing implement of some sort would be criminal.
I wear either my ScottEvest or jacket all the time which has convenient pockets for both. Often carry a small notebook as well.

Yes, I know I can keep notes on my smartphone or dictate a voice note but I can't draw a decent picture with it.
