I have really enjoyed reading the posts, thought I'd put my 2 cents in. I always have an 18 gauge sterile needle in all of my kits. It's great for many uses and doesn't take up much space.Ask your friendly medical or vet professional.
I found a saw made by Allway tools, it's small, lightweight and has 2 blades. Runs about $3 or $4. This is the FS2 model and can be seen on their site www.allwytools.com It's too big for a small tin sized kit, but will go anywhere else.

I saw one of those in WalMart recently, and thought that it looked good for my vehicle supplies. Sadly, it is too large for an altoids tin, you are right about that.
