I received the same e-mail and likewise started to fill out the form. Then I got to thinking, Ebay always states they would never ask for personal info, so I too back out of the form. I tried to find an Ebay e-mail address to send an alert, but never found one. Could you please either post the one you found or PM it to me, so I can also report the matter. Thanks Pete
