I'm thinking about 1-2 laminated 'instruction' cards for the wallet -- what would you put on them? More to help jog the memory than full instructions....

Your name, birth date, any meds you may be allergic to, contact info for family, any stints or other metal you may have in you, any medical conditions you may have. Your insurance info, and if you want a DNR order, if you are an organ donor

I would also include my car's license info. I have this fear that if one of my vehicles ever gets stolen, I won't be able to tell the police what the tag number is. Along the same lines, I might include the phone numbers of my credit and debit card companies. Alas, that won't do you much good if it's kept in with the wallet with the cards that just got stolen.

I might also consider the phone number of a preferred wrecker company or AAA and even a taxi company -- though usually a call to 411 can provide that.

A statement regarding a DNR in this format would, I suspect not be recognized in most, if not all states. Most Advanced Directives require a statement as to what advanced medical procedures are or not desired and generally the document must be witnessed and possibly notarized. Many states use a DNR only if the patient has a terminal condition and generally must be under a physician’s care that signs and authorizes the DNR.

In Maryland, only a state (any state’s) issued DNR is recognized in the pre-hospital setting, Advanced Directives are not acceptable. Maryland DNR has two options, one for palliative care only (i.e. O2, position of comfort, pain management), the other for full care up to cardiac and/or respiratory arrest, (i.e. I.V.s, medications, etc., but no electrical or ET interventions).

Oh - actually I thinking more instructions for ME -- things like first aid basics, knots, that sort of stuff...

I think it would be situational or person specific. At one time I had 'info cards' of key computer sites, emergency team memeber contact info (cell # home # etc.) and some specific passwords and/or emergency access info.

Now I've got my DayTimer with home info, car info like plate number, VIN and the like and a bunch of passwords and contact site info that I can use to figure out how to get on computer sites I may need (done up so they make sense to me but not to someone else). I keep that updated each year with a full rewrite each January.

  • All the phone numbers that are stored in your cellphone - in case your cell phone dies
  • All your credit card info and contact numbers for cancellation (obviously this should not be stored in your wallet)
  • Emergency contact info - who should be called when the medics go through your wallet and you're unconscious. Relative? Physician? Both?
  • Your unique medical conditions that medics should know about (allergies, etc)

    Things you want to remember that are specific to you...
  • fishing or hunting season dates and size and bag limits for your state
  • knot diagrams
  • tarp shelter configurations
  • helicopter - ground to air signals X Y -> F I etc
  • local football team schedule
  • The possibilities here are endless - and personal/unique to you
