I think this might be the pouch you are thinking about. Pock-its Utility Holster I have no connection with the company other than as a consumer. It took several years but I wore one out. I'm now on my second one.

The moleskin – are you looking in first aid, or in foot care? Our Safeway and Walgreen's have in foot care (near the Dr. Scholl's etc. products). For my 32-oz. Nalgene, I'm using a Snow Peak Titanium Trek 700. Technically, it's not a cup, but a pot. It comes with lid and mesh bag (only used for hiking trips). Perfect size cook-pot for small hiking stoves, and fits the Nalgene perfectly. Snow Peak Trek 700 If you really want minimal weight/bulk for Leatherman/HDS light side-by-side, you might consider having a custom-fitted Kydex sheath made (guessing $25-30). I have been using a dual pouch from Western Case Company for a long time, and am very happy with it. I also use their cases for my cell phones. They use the strongest belt clip I have ever seen, you will not lose one of their cases, which can not always be said for other brands of clip-on pouches...

I just found and ordered two GSI stainless cups that fit under the 32oz. Nalgene bottles from basspro.com With shipping they were only approx. $13.00. Can't really recommend them yet...but they look good! http://www.basspro.com/webapp/wcs/stores...t=SearchResults

I have one of the Nite Ize pouches, I carry my cell phone and a small light in it. I've had it for over a year and it's held up very well. I would not hesitate to purchase again.

If you wanted something a little nicer and more durable for both a light and multi-tool, many suppliers like Galls sell firearm magazine/flashlight holders made from leather. I have carried a Leatherman and Surefire in one of these.

Just a quick word on the moleskin. I have found the Dr. Scholl's to stay on the best. Don't try buying them on surplus sites, they will send you the GI issue. (Trust me I've tried them in military operations and they are terrible) I would also stay away from any other blister remedies sold in stores. i.e., blister packs. They are bulky on the foot and rub off easily.

Would Maxpedition double-wide multi-tool/flashlight sheath work? Measurements look compatible with the LM Charge.

I use .45cal double magazine pouch made out of leather but the canvas ones also work very well. The leather ammo pouch requires me to put a little paper filler into the bottom so that the Leatherman will ride high enough to be easily retrieved when needed. My buck 110 fits into the pouch perfectly.
I wrap some nylon thread around and between the two pouches.
I also hang a halyard clip from one of the two belt loops. I hang my hat or tools from this clip, depending on what I am up to at the moment.
