I guess from the Japanese Fukushima nuclear reactor disaster that one of the main lessons that has become obvious was that US designed GE and Westinghouse Boiling water reactor designs (along with the old Soviet RBMK reactors) are not safe and can fail catastrophically. This has been secretly known since they were built but the American public was essentially misinformed about the safety of these designs. i.e. it was known that even an inappropriate use of a candle could potentially lead to a catastrophic nuclear power plant emergency.
VLE ( very large event) planning is based on what if's, assumptions and outright guesses -- and can be off by a factor of 100. Katrina is a good example; one thing led to another and another...And there is only so much that can be done. Wall have limited height, money is limited.
